Check your Balance or Pay by Phone

You may check the balance of your account or pay by phone 24 hours a day, 7 days a week by calling 1-800-225-0953 and following the prompts. Please be prepared to supply the patient’s account number and date of birth.

Call to Speak to a Representative

You may call 1-800-225-0953 to speak to one of our Patient Account Representatives in our Customer Service Center, Monday - Friday 7am – 7pm CT. If you are the patient, are authorized to act on behalf of the patient or represent an insurance company, please be prepared to supply the patient account number, date of birth, and last four #’s of the patient’s Social Security Number or the patient’s name, location of treatment, date of service, date of birth, and the last four #’s of the patient’s Social Security Number. If you are a parent or guardian and the patient was over the age of 18 at the time of service our office must have written or verbal permission from the patient prior to any discussions.

Attorney Requests Only
Log in to ChartSwap.com and upload a valid Letter of Authorization (LOA).
A valid LOA will need to include the provider’s name, date(s) or date range for the request, patient’s date of birth and/or the patient’s SSN, an expiration date of the LOA, and must be signed and dated by the patient or the legal guardian of an underage patient. You will be notified by email once your request has been fulfilled. If you are following up on a request previously submitted through ChartSwap.com, please log back in to ChartSwap.com for the status of processing your request.
